Avatar not moving with me

Had this happen a few times, I will be moving between 2 portals but my avatar will become stuck and not advance with me.
If the portal I am moving to is within visual distance when I physically get there I can click on the portal and interact with it normally like the avatar is there, but when I finish it is back at the last portal. So the scanner thinks I'm at the new portal but shows me at the old one.
It seems if I have a key to the portal I'm at I can use it to get the avatar to move again but it will then stay again at the new location. Only way to fix is to restart.
I'll try and get video and screenshots when it happens again.

Here is video of what is happening.
Seen many people with the same issue in local chats, me included. Happens several times during the day, cause lots of prime restarts to fix it. Started happening in the beginning of August
That's when it started happening to me too. Hopefully with this new patch it stops
I regularly face this issue in certain locations.
Experienced this as well, if you move long enough the map will start disappearing, until you are in a black void. Seems to be just visual, restart fixes it.
