Niantic released new portal criteria. All previous criteria, AMAs and guideline clarifications are invalid.

There is a new rejection criterion "Abusive Location" for "Locations that are intentionally and strategically placed to provide advantage to a single player or collective group". Does that apply to PITA portals on in locations that are not accessible to the general public?

Portals, at enterprises with access control (e.g. factories), should now be rejected?

Old clarifications aren't invalid. This new "portal criteria" is really just the same thing they've been saying all along just in simplified language.
