Head over to support.ingress.com and open a ticket under the Gameplay Help -> Kinetic Capsule Issue category. If you have it from screenshots or the like, also keep your capsule ID ready.
Do note that the Rare Kinetic Capsules, the purple ones, are destroyed after 3 uses, that is intentional and not a bug.
