I've had this happen once, about a year ago, but only a single quantum capsule, disappearing directly from my inventory.
The content was VRMH and VRHS that I'd crafted during a kinetic capsule 2km event.
It happens with me with one also. Sometimes it was because I dropped it unwillingly with my screen unlocked. And yes I have read a story that someone did lose a lot of quatums with valuable keys and so. It is an unwanted feature of the scanner in my eyes.
