Suggestion: OPR: Portal Edit Reason Field

Problem: It has been frustrating to make corrective changes to the Portal Network because the single answers in the submission do not adequately explain the reasons for those changes.

Solution: Include a short form for the reason so the reviewer has a better understanding of why the portal should be removed / altered.
