It marks the portals you've captured, visited and/or scanned. First time you captured that portal would have been a unique capture and incremented your Pioneer badge. Subsequent captures don't affect the count or map layer.
I'm having the same problem. While the majority of the portals with no red ring around them are portals I have not captured, there are portals I know I have captured dozens of times with no red ring around them.
In other words, it's not 100% accurate. What could be the explanation for this?

Wow, this pretty much came out of the blue, but it's great! I've been lately finally approaching platinum tier on Pioneer and Explorer badges, focusing heavily on areas comparatively near I live for obvious reasons. Now I don't have to just rely on my limited human memory.
There are loads of portals that I know I have visited and captured, but not showing as such on the map. Some of these portals I have regularly captured on a daily basis. Some, multiple times per day, but showing as uncaptured. Is this functionality bugged?

Agreed, time to go experiment and see if my upc count goes up or not.
Yes, a view that shows only the portals you haven't captured would be nice for those areas where only a few portal are uniques.
With apologies for going "This is great!" to "can we also have this? " within about 30 minutes.

I had the same issue with a few portals. Visited and/or captured, but not showing up on the map.
Try refreshing the intel map. Sometimes this helps.
